* Supporting documents might include copies of:

    • any medical report which explains your injury or illness (if any), how it affects you (eg. your ability to work, medications or treatment you might need), your likely recovery time and long term prognosis;
    • payslips, income tax returns, bank statements and/or correspondence which evidences your current income (including any pension or Government assistance payments);
    • statements or correspondence from your bank/lender or landlord/real estate agent which show your (weekly/fortnightly/monthly) mortgage or rent expense;
    • invoices/accounts of any particular debts you need help to pay (eg. electricity or telephone bills, credit card bills);
    • quotes for any particular piece of equipment or service for which you would like to request funding assistance (eg. wheelchair or other mobility aid, home access or vehicle modifications, carer services, temporary childcare services).

The Trust Secretary or NSW Jockeys Association will then contact to you:

  • confirm receipt of your Application;
  • to ask any additional questions and request any further information needed to process your Application; and
  • to confirm the date that the Trustees of the NSW JOCKEYS TRUST will be meeting to consider your Application.

The Trust Secretary will contact you after the Trustees’ Meeting to advise you of the outcome of your Application.
